I've gotten many many compliments from other musicians with a sharp eye after I replaced my broken nameplate with my own custom.
On most amplifiers, simply unscrew the old name plate and re-use the screws for your new custom plate.
The name plate measures approximately 6" x 2" x 0.13". Exact size may vary by text length.
Mounting holes are 0.11" (2.8mm) with a 0.23" (5.7mm) countersink/top diameter. Custom screw sizes may be available upon request.
