This is in full working order except for the headphone output, which only outputs the right-side of the signal. it was like this when I brought it 3 years ago (I didn't need a headphone output).
I've had it for 3 years now, and been using it as an amp attenuator and FX loop for pedals, selling as I'm using digital more and more and don't need to attenuate a big amp any more.
Cosmetically, there is a small bend on the bottom right front plate (pictured)
